I have an old Honda civic, 1997, low miles and in good shape. The tires are from 51st week of 2004, but have low miles. We are planning a long drive, about 4000 miles, change them out or keep them?

A lot would depend upon some other factors besides mileage. Has the vehicle been parked for a long time or in use as a daily driver? What load do you intend to have, one person or four plus gear for a three week camping trip? What speed do you intend to drive? What is the condition of the roads you plan to drive, all paved or some gravel or unimproved?
Since you asked the question you must have some concern. If it were me and my budget would stand it – I would replace the tires, relax and enjoy the trip.
